16 Jun 1971: 01521343E Commandant John Skerten Innes from the Army Staff Corps was attached to the Joint Combat Liaison Mission, Luanda when he suffered a fatal heart attack while on duty in Luanda. He was 42.
16 Jun 1976: 7226894BT Rifleman Clifford Donald Holland from the Durban Light Infantry was killed at Ruacana when he was struck by bullet resulting from an accidental discharge of a fellow soldiers rifle. He was 19.
16 Jun 1977: 66490269BT Rifleman Stephen Jones from Regiment Schoonspruit died from a gunshot wound accidentally sustained at the Oshivello Training Area. He was 27.
16 Jun 1984: Rifleman Johann Jacobus Hurter from 1 Parachute Battalion was killed in a private motor vehicle accident while on weekend pass. He was 24.
16 Jun 1984: Special Sergeant N. Nghifino from the South West Africa Police Counter-Insurgency Wing: Ops-K Division (Koevoet) was Killed in Action during a contact with SWAPO/PLAN insurgents in Northern Owamboland. He was 32.
